Sliding Fee Discount Program

Availability of Discounts

The Sliding Fee Discount Program is available to all patients, including those who are uninsured or underinsured.

Eligibility

Discounts are based on:

• Total household income
• Household size

Eligibility is determined in accordance with the current Federal Poverty Guidelines.

Discount Structure

Patients who qualify for the Sliding Fee Discount Program will receive services at reduced fees based on their income level.

Patients with incomes at or below 100% of the Federal Poverty Guidelines will be charged a nominal fee for services.

Patients with incomes above 100% of the Federal Poverty Guidelines may receive partial discounts.

A full schedule of fees and discounts is available upon request.

Services Covered

The Sliding Fee Discount Program applies to services provided directly by Preferred Medical Group which may include:

• Primary medical care
• Behavioral health services

Services not provided directly by the health center (such as outside laboratory services, imaging, or referrals to specialists) may not be eligible for discounts.

How to Apply

Patients may apply for the Sliding Fee Discount Program at any time.

To apply:

• Complete a Sliding Fee Discount Program application
• Provide documentation of income for all household members (such as recent pay stubs, tax returns, or other proof of income)
• Submit the application in person at any clinic location or by contacting our staff for assistance

Applications will be reviewed promptly, and patients will be notified of their eligibility.

Additional Information

Sliding fee eligibility applies only to services provided at Preferred Medical Group.

Eligibility must be renewed annually in accordance with health center policy.
